Tijuana in 2026

What the U.S. travel advisory says about Tijuana

The U.S. State Department rates Baja California Level 3, “Reconsider travel,” due to terrorism, crime and kidnapping. Mexico as a whole is Level 2. That is the advisory issued May 29, 2026.

The Tijuana wording is specific. It says “There are high homicide numbers in the non-tourist areas of Tijuana” and “Most homicides appear to be targeted.”

It also places no extra travel restrictions on U.S. government employees in Tijuana, Ensenada or Rosarito. The only restricted area in the state is the Mexicali Valley. Read the full advisory.

Mexico City, where our clinic is, and Quintana Roo, home to Cancún, are both Level 2 on the same advisory. If you live in Southern California, Tijuana is a drive. Mexico City has nonstop flights from 14+ U.S. cities.

Getting to treatment: Tijuana or Mexico City

  • Tijuana: from San Diego, the main crossing is San Ysidro, on foot through El Chaparral or by car.
  • Tijuana’s city government sells medical Fast Lane passes to registered medical providers, who issue them to patients for the drive back north at San Ysidro or Otay. Ask a Tijuana clinic whether it is registered.
  • A land entry to Mexico longer than 7 days carries a visitor fee: 983 pesos, about $53, in 2026.
  • Our clinic: a private driver meets you at Mexico City airport (MEX), Terminal 1, with a name card, and drives you back on day 7.
  • Level 3Baja California, U.S. travel advisory of May 29, 2026
  • Level 2Mexico City, on the same advisory

Who we treat

Who we treat, and who we turn away in writing

We treat dependence on opioids (including fentanyl, Suboxone and methadone), alcohol, cocaine and methamphetamine. We turn away anyone whose heart, medications or recent use makes the dose unsafe, and we tell you which rule applied.

Opioids

Heroin, fentanyl, oxycodone and other short-acting opioids. Suboxone and methadone need a planned transition before travel, set by our psychiatrist with your prescriber.

Alcohol

Medically supervised detox comes first. Dosing waits until electrolytes and the liver panel meet our written thresholds, so for some people the stay runs longer than 7 days.

Cocaine and meth

Stimulants strain the heart, so screening goes further: a longer cardiac history, a lower QTc threshold and cardiology clearance when indicated.

We say no to:

  • Structural heart disease, heart failure, cardiomyopathy or a prior cardiac event
  • A QTc or conduction finding above our written thresholds that isn’t caused by something correctable
  • Benzodiazepine dependence as the main problem
  • Pregnancy or breastfeeding, and anyone under 18
  • Anyone who wants the dose without the screening, the 7 days or the follow-up

We say not yet to:

  • A washout that isn’t complete, or use during the washout window
  • Potassium, magnesium or liver values out of range, until corrected and rechecked
  • Open xylazine-related wounds that need treatment before travel
  • A QT-prolonging medication your prescriber hasn’t agreed a plan for

Both answers come in writing, with the reason. Any deposit is refunded in full. Telling us the truth about what you’ve used is never a reason for a no.

Fentanyl

Fentanyl means a longer wait before the dose, in Tijuana and here

Fentanyl stores in fat and clears on its own schedule, and street fentanyl is often cut with sedatives that ibogaine doesn’t treat. Dosing before it has cleared risks withdrawal on the day the heart is under the most strain.

In a public statement dated January 21, 2026, a Baja California ibogaine provider disclosed a patient death in its detox program. It named unknown, changing adulterants in street fentanyl as a risk, lengthened its detox stays and set a 21-day minimum stay for anyone using fentanyl.

We don’t publish a fixed number of days. Our physician sets your washout in writing before any deposit, based on how long and how often you’ve used, by what route, and what else is in your supply.

If you use during the window, tell us and the date moves, with no penalty. Whatever clinic you choose, don’t push for a shorter wait.

How our fentanyl screening runs

  1. Your pattern

How long, how often, by what route, and when you last used.

  2. What else is on board

Xylazine, medetomidine, benzodiazepines, gabapentin, stimulants and every prescription.

  3. Your EKG and labs

A 12-lead EKG plus potassium, magnesium and a liver panel, ordered where you live and read by our cardiologist.

  4. A written window

Your washout length in writing, with the conditions that would extend it.

  5. Day 1, in person

Repeat EKG and labs on arrival. If the numbers moved, the answer changes here, not in the dosing room.

Our protocol beside the FDA’s proposed ibogaine trial design

On October 6, 2026, the FDA asked for public comment on how U.S. clinical trials of ibogaine should be designed (docket FDA-2026-N-10429, comments close November 20, 2026). It is a request for information about research trials. It doesn’t approve ibogaine or set rules for clinics.

Here is our published protocol beside it, row by row.

FDA column: Federal Register, October 6, 2026. Our column: the program, medical team and fentanyl pages of this site.

FDA proposed ibogaine trial design compared with the Ibogaine Treatment MX protocol
ItemFDA proposal for trialsWhat we do
SettingAn inpatient unit equipped to manage a life-threatening arrhythmia: an ACLS-trained, physician-led team, a bedside defibrillator, pacing and ventilator supportA COFEPRIS-licensed facility with a crash cart, oxygen and a defibrillator at the bedside, and a written transfer agreement with a private hospital 9 minutes away
DoseStart at no more than 10 mg/kg, and participants can be dosed 1 at a time1 supervised dose on day 3 of 7, at the dose our physician sets for your case
Heart cutoffExclude a baseline QTcF of 430 ms or more. The FDA asks for comment on that cutoff and lists women as more susceptibleA cardiologist reads your EKG against a written QTc threshold you’re told before you pay. Stimulant use gets a lower threshold
ElectrolytesCorrect low potassium, calcium or magnesium before dosingPotassium, magnesium and a liver panel before you fly, repeated on arrival. Values out of range mean not yet, until corrected
MonitoringContinuous monitoring until the rhythm is normal and QTc is near baseline, which could take 36 hours, after a pre-dose baseline recording such as 24 hoursContinuous 12-lead ECG and pulse oximetry from before the first dose, through the session and overnight, then a morning EKG each day until discharge
StaffAt least 2 staff per participant during acute effects, 1 of them a licensed mental health professionalA cardiologist, an emergency physician and an ACLS-trained nurse in the room, 1 nurse to 1 patient. We differ here: our psychiatrist plans your care before and after, but isn’t in the dosing room
MedicationsStop methadone, buprenorphine, QT-prolonging, serotonergic and CYP2D6-interacting drugs firstSuboxone and methadone need a planned transition before travel, and SSRIs are reviewed 1 by 1. Our psychiatrist sets the plan with your prescriber, and dosing waits until it’s complete
DischargeAfter physician approval, for example a 12-lead ECG at 48 hours without QTc prolongation, with a trusted adult escorting you homeDay 7, after a final EKG and physician sign-off. Our driver takes you to the airport

The 2026 safety data

What the newest data says about ibogaine deaths

Deaths after ibogaine are rare. In a 2026 dataset of 19,071 patients, every death within 72 hours was in a patient treated for opioid use disorder.

  • 0.03%6 deaths during or within 72 hours among 19,071 patients at 11 clinics, 1996-2026
  • 0.06%6 of 10,382 patients treated for opioid use disorder
  • 0 of 8,689patients treated for other reasons

The source is a preprint posted June 17, 2026. It hasn’t been peer reviewed.

All 6 patients who died were men aged 45 to 49. Where the dose was recorded, it was 14-15 mg/kg.

Read it with 2 caveats. The clinics reported their own deaths, and nobody checked them independently. 1 co-author holds equity in an ibogaine clinic.

The same paper counts 48 reported ibogaine deaths worldwide from 1990 to 2026, 13 of them in Mexico.

Where our protocol meets that data

  • Continuous 12-lead ECG and oximetry from before the first dose, through the session and overnight
  • A cardiologist in the room for the dose, and an emergency physician on site overnight
  • A morning EKG every day until discharge on day 7

Ibogaine still carries real risk. This data shows where that risk sits, so you can ask any clinic what it does about it.

Research

What the 2026 research shows about results

The strongest new outcome data is Stanford’s 12-month follow-up of veterans treated in Mexico, published August 12, 2026 in Translational Psychiatry. It tracked PTSD, depression and anxiety after brain injury. Addiction wasn’t part of it.

30 male special operations veterans with traumatic brain injury had 1 ibogaine treatment session with IV magnesium. 25 completed the 1-year follow-up.

  • 84%still in PTSD remission at 12 months
  • 66%still in depression remission
  • 61%still in anxiety remission

Those figures cover the veterans who went into remission right after treatment. There was no control group, and 23 of the 25 used another psychedelic during the year. Read Stanford’s summary.

For addiction, no large randomized, placebo-controlled trial has shown that ibogaine works. The FDA request above is a step toward U.S. trials, and the FDA has also allowed an early-phase study of noribogaine, ibogaine’s main breakdown product, for alcohol use disorder.

How it works

The 6 steps at our clinic, from first call to day 90

The first 2 steps happen before you buy a flight. Any step can end in a no, and nothing is charged before a physician says yes in writing.

  1. Before travel

    Screening on paper

A 12-lead EKG, potassium, magnesium and a liver panel ordered near your home. Our cardiologist reads them before you buy a flight.

  2. Before travel

    Washout planned

If you take Suboxone, methadone or an SSRI, the taper is scheduled with your prescriber. No dosing until it’s complete.

  3. Days 1 to 2

    Arrival, go or no-go

A driver meets you at MEX Terminal 1. Repeat EKG and labs. If anything has moved, dosing waits or is declined.

  4. Day 3

    The dose

Continuous ECG and pulse oximetry, with a cardiologist, an emergency physician and an ACLS-trained nurse in the room. Crash cart and oxygen at the bedside.

  5. Days 4 to 7

    Telemetry and recovery

Overnight monitoring, a morning EKG and daily physician rounds. A written summary for your doctor at home before you leave.

  6. Days 8 to 90

    Follow-up

Weekly calls to day 30, then calls at days 45, 60 and 90. A referral list of U.S. clinicians near you.

FAQ

Questions about ibogaine treatment in Tijuana

Short answers. Anything else, ask on the call.

Is Ibogaine Treatment MX in Tijuana?

No. Our licensed medical facility is in Mexico City. The address is given when a visit or a stay is arranged.

Is ibogaine legal in Mexico?

Ibogaine is unregulated in Mexico. It isn’t on the psychotropic schedules of Mexico’s General Health Law, and it isn’t an approved medicine there. In the U.S. it is a Schedule I drug, which is why U.S. patients travel.

Is Tijuana safe to travel to for treatment?

Baja California is Level 3 on the U.S. State Department advisory issued May 29, 2026. The advisory ties Tijuana’s high homicide numbers to non-tourist areas and says most appear targeted. Mexico City is Level 2.

How much does ibogaine treatment cost?

Our 7-day program is $12,500, all-inclusive, with 90 days of follow-up. Tijuana clinics price differently, so ask each one for an itemized quote and compare it line by line.

How long is the wait before dosing if I’ve used fentanyl?

Our physician sets your washout in writing before any deposit, based on your pattern of use. At least 1 Baja California provider now sets a 21-day minimum stay for anyone using fentanyl.

I’m on Suboxone or methadone. Can I be treated?

Often, yes. Both need a planned transition before travel, set by our psychiatrist with your prescriber, and dosing waits until it’s complete. The FDA’s proposed trial design also calls for stopping both first.

Can my family come?

Yes. Family can join the screening call, visit the facility before you decide, come during daily visiting hours and be part of the 90 days of follow-up.

What do I need to travel to a Tijuana clinic?

A valid passport, and check the current rules for your route. A land entry to Mexico longer than 7 days carries a visitor fee: 983 pesos, about $53, in 2026.

Does ibogaine work for addiction?

No large randomized, placebo-controlled trial has shown that it does. Published studies are small, mostly without control groups, and measure reduced use, not a cure.
